East Surrey Place is one of four ‘places’ within Surrey Heartlands.
A place-based partnership is a collaborative arrangement made by the organisations responsible for arranging and delivering health and care services in a locality.
Closer working together at place level will enable East Surrey to provide improved patient care and work effectively for the benefit of residents – by offering joined up services that meet and respond to local people’s needs; enabling them to create and live healthy lifestyles; and supporting them to take responsibility for their health and wellbeing.
Organisations, professionals and clinicians will work together to create positive health outcomes for local communities.
Our partnership
The East Surrey Place partnership includes Surrey Heartlands ICB, Surrey and Sussex Healthcare NHS Trust, GP Federations Alliance for Better Care and Dorking Healthcare, five Primary Care Networks, First Community Health and Care (NHS community services) and Surrey County Council, along with Reigate & Banstead Borough Council and Tandridge District Council, Surrey And Borders Partnership, South East Coast Ambulance, St Catherine’s Hospice and other voluntary sector partners.
While many of the partners also deliver services to geographies beyond East Surrey – such as Sussex or other parts of Surrey – this partnership concerns services and outcomes for the population of East Surrey in particular – the geography previously encapsulated by the East Surrey Clinical Commissioning Group. There will be similar partnerships for other Places and organisations may well be involved in more than partnership.
There are already many examples across East Surrey of teams working together brilliantly across organisations and in partnership with the communities they serve to deliver existing services, design news ones and continuously improve our offer to local people. Developing East Surrey as a Place will help to build on these successes and help us to tackle current challenges.
Closer working together at Place will enable us to provide joined up services that meet and respond to residents’ needs; enable and empower our residents to create and live healthy lifestyles; and support them to take responsibility for their health and wellbeing.
Engaging with our population
In East Surrey we believe in the power of partnership; organisations, professionals, clinicians, and our people working together to create positive health outcomes for local communities and building the future we all deserve.
We see the person, and the health outcomes we create with them, as being at the centre of everything we do. We want people in East Surrey to get the care and support they need as early and as close to their home as possible. This means that we recognise and prioritise the importance of engaging with our communities to ensure the services provided locally support prevention and early intervention in the management of a range of conditions, and in keeping East Surrey healthy and well.
We want to hear people’s views at every level of health from design to delivery. If you would like to get more involved in the way services are delivered locally, please get in touch.
